As mandated by Missouri State High School Activities Association, student-athletes are not eligible for tryouts or competition until they have an annual physical and complete the MSHSAA Physical Exam Form. When completed, the form must be submitted electronically to the PRIVIT website. All other forms shown under the athlete's account must be completed. To ensure proper completion, please refer to either the New Parent Instructions or Returning Parent Instructions. Student-athletes are not allowed to participate in tryouts or contests until all forms are completed and submitted correctly online by Friday, Feb. 26. MSHSAA Bylaw 3.8.1 and 4.5.4 now allows a physical exam certification by a medical professional (MD/DO/ARNP/PA/Chiropractor) and completed on or after August 1, 2018, to be valid for a duration of two years from the date of issue, unless a physician specifically notes on the PPE that the physical is only valid for one year. While the MSHSAA PPE Form is now valid for a two-year period from the date of the physical examination, all other pre-participation documents on PRIVIT must be completed/updated annually for every student-athlete.
NOTE: It is imperative to use the appropriate Parent Instructions and proper submission. No paper forms will be accepted and will be returned to the student-athlete. It is required that all athletes must have a current baseline test for the school year on file via the XLNTbrain website prior to the start of tryouts. Tests must be repeated each school year by all student-athletes.Baseline tests are easily completed at home (recommended) by your daughter, and take approximately 40 minutes to complete. Please click here for the Athlete/Guardian Instructions on how to register and complete the test. Parental enforcement of this requirement helps prevent student-athlete ineligibility. Daily COVID-19 Check-in Form
Student-athletes are required to fill out the form linked below each day before tryouts and practice. A student’s temperature must be taken daily and logged into this form along with a few other health-related questions. This form is mobile-friendly and takes less than a minute to complete. Team coaches will utilize the responses to this form to take daily attendance.
